Complex Endovascular Aortic Repair Successfully Performed on Octogenarian at Father Muller
Mangaluru: A team led by Dr. Anand K T, Cardiothoracic and Vascular Surgeon, and Dr. Ariharan K, Interventional Radiologist, at Father Muller Medical College Hospital successfully performed a complex endovascular repair of an abdominal aortic aneurysm (AAA) in a patient over 80 years of age. This marks the institution’s third complex endovascular aortic procedure this year.
The patient had an abdominal aortic aneurysm associated with significant kidney disease. Advanced age, renal dysfunction, and markedly tortuous vascular anatomy made the procedure particularly challenging, requiring detailed pre-procedural planning and careful intra-procedural management.
Advanced Endovascular Treatment
The patient underwent total endovascular aortic repair using specialised aortic endografts, allowing the aneurysm to be treated through a minimally invasive approach.
The procedure was performed in the hospital’s advanced biplane Cath Lab, which provides high-quality, real-time imaging from multiple projections. The facility enables precise navigation of endovascular devices and accurate deployment of complex vascular grafts, particularly in anatomically challenging cases.
Due to the patient’s complex anatomy and medical comorbidities, the procedure required meticulous technique and lasted more than seven hours.
Anaesthesia was managed by Dr. M M Chetana Anand, Consultant Cardiovascular and Neurovascular Anaesthetist. Given the patient’s advanced age, renal dysfunction and the prolonged duration of the procedure, careful anaesthetic planning, continuous haemodynamic monitoring and meticulous peri-procedural management were essential to ensure patient safety.
Despite the complexity, the procedure was completed, and the patient made an uneventful recovery. The patient was discharged in good condition three days later.
