In his welcome address, Dr Anand Venugopal, Medical Superintendent of KMC Hospitals Mangaluru said, “Piles (Hemorrhoids) have always been a taboo subject, which is why many people are surprised to learn that the majority of the population will develop a haemorrhoid at one stage in their lifetime. To receive the best advice and treatment for a haemorrhoid you will need to seek help from a medical professional, which means putting your embarrassment aside and talking about your condition in detail. And today we have two experienced doctors who have good knowledge on this subject, and I request all of you to make the best use of the awareness talk and also go for the screening. I thank Dr Manohar Pai and Dr Shibumon for taking the initiative in conducting this awareness programme on Haemorrhoids”.

Briefing about Piles through the audio-video presentation, Dr Manohar Pai said, “Hemorrhoids” or “piles” are like Cushions, composed of Arterio- Venus channels and Connective tissues, in the anal canal, which helps in regulation or control of stools. Due to excessive stress sometimes they become pathological or “Piles “with symptoms of Swelling and inflammation. Piles can be internal and external & initially can be characterized by slight pain and itching sensation while defecation, later on, it becomes worse with daily bleeding and pain along with external or internal lumps or lobules like structures. Depending on the origin and morphology Piles or Hemorrhoids are broadly classified as:- Internal Piles (Hemorrhoids): Occurs in Anal Canal and Rectum; and External Piles (Hemorrhoids): Outside Anal Canal near anal opening.

He further said, “Piles don’t always cause pain or other symptoms. But other symptoms are -Bleeding along with bowel movement – you may see blood on your feces; A lump in or around your anus; A feeling that your bowels haven’t emptied completely; Watery discharge or mucous from anus, or leaking feces; Itchy or sore skin around your anus; Pain and discomfort during or after a bowel movement. These symptoms can be caused by problems other than piles, such as inflammatory bowel disease, anal cancer, bowel cancer and anal fissure (tear) so always consult your Doctor in such conditions.”

“Regarding Treatment of Piles- Piles are common and often mild and get better with some simple lifestyle improvements. There are numerous things that you can do for effective treatment of piles. Self-help or self-treatment of piles patient:- By making diet and lifestyle changes, you can often help to relieve symptoms of piles. For example, eating a high-fiber diet like raw fruit and vegetables, cereals and fiber supplements will make your stools softer and ease bowels. Drink enough fluid to keep hydrated, and don’t have too much caffeine or alcohol. Try not to strain when you’re passing bowels. Regular warm sitz baths twice or thrice a day may relieve some of your symptoms. Some experts think there isn’t enough evidence that sitz baths help to treat piles though. If you’re passing hard feces, a fiber supplement or mild laxative will soften your feces. But in the hospital, there are other treatments like Banding; Sclerotherapy; Infra-red coagulation or laser treatment of piles; Bipolar diathermy and direct current electrotherapy treatment of piles; and Doppler-guided haemorrhoidal artery ligation. However, your doctor will advise you the benefits and risks of the procedure recommended for you” added Dr Pai.

Dr Pai further said, “Most piles don’t need surgery for piles treatment. Surgical treatment of piles is required in case of severe or older piles when all other treatments haven’t worked or your piles keep bleeding. There are different types of surgery, like Haemorrhoidectomy- In this technique any pile masses, causing problems are carefully removed; Stapled Haemorrhoidopexy- the area of tissue with piles is stitched higher up your anal canal. This means your piles or hemorrhoids won’t come out your anus anymore and will shrink gradually. The type of surgery that your doctor recommends for you will depend on the size and number of piles you have.”

“The causes for Piles are- Modern Lifestyle, Junk food, Stress, Improper eating habits, Indigestion and Prolonged Constipation, Ageing, Diarrhoea lasting a long time, Long-term coughing, Having a family history of piles, Excessive Oily, Spicy, fatty & Non-Veg food, Lack Of High Fiber Diet, Long sitting working hours, Prolong use of Antibiotics & Allopathic drugs, Pregnancy & Overweight, and Excessive Heavy weight lifting . But small changes in lifestyle can make your bowels regular and faeces soft, thus preventing constipation and piles. Eat plenty of fiber-rich foods such as fruit, vegetables, whole grain cereal, wholegrain rice and pasta, nuts, beans and pulses; Drink plenty of fluids and have lite but regular exercise; Avoid caffeinated drinks & Alcohol; Avoid sitting for long time in the same place.” added Dr Manohar Pai.

Dr Shibumon describing Minimally Invasive Procedure for Hemorrhoids (MIPH) treatment for Piles said, “Most people with piles do not complain of any symptoms until the condition becomes quite severe. Painful or severe symptoms which interfere with the day to day activities and decrease the quality of life will need medical advice. Identification of causative agents and proper, timely treatments will definitely result in a positive outcome.Out of the various types of medical and surgical interventions available to treat Piles, MIPH or Haemorrhoidectomy is known to be very popular since it acts an alternative approach to surgical interventions made for piles.MIPH, has several advantages over surgical treatments such as minimal pain, minimal blood loss, quick recovery and reduced hospital stay. In fact, patients will be able to return to their normal daily activities within a day. KMC Hospital Attavar is introducing a special package of Rs. 24,970/- for MIPH treatment at General Ward. Those who attend the Awareness programme and free screening camp can avail this facility and can get treatment for Piles painlessly.”

He further said, “Piles (Hemorrhoids) are swellings that can occur inside and around the anal passage and the anal canal. When the veins which carry blood vessels are stretched, they can become very irritated and cause swelling, mucus, itching, bleeding and discomfort. However, due to the embarrassing nature of the condition, it is not widely discussed and some people are unwilling to see a doctor. As a result, many people are left misinformed and confused about what hemorrhoids are, where they come from and, most importantly, how to get rid of them once and for all”.

Many in the audience posed queries at the two doctors and clarified their doubts. Rakesh, In charge-Marketing of KMC Hospital Attavar introduced the speakers of the day and proposed the vote of thanks.
